Our Mission:
The Mission of Elder Care Management is to improve the quality of life for
aging adults. Elder Care Management offers the evaluation and coordination
of services necessary to promote the continued independence of elderly
adults and for those needing additional services.
Care Planning and Care Plan Management will be tailored to meet the
client's needs.
There will be guidance in the selection of supportive services to maximize
the seniors’ comfort, health and safety while promoting independence
through cost-effective solutions.
